HOW TO ADMINISTER WAIVERS –
INSTRUCTIONS FOR EVENT HOST(S)
For events that involve physical activity York requires that waivers be administered by the event host:
Waivers are to be printed double sided and are to be administered at least one week prior to the initial event date as indicated on the waiver. Waivers are administered in advance to allow the participant the opportunity to read, understand, sign and return the waiver. If a participant does not wish to sign a waiver or alters a waiver in anyway, they are not to participate in the event. Waivers are to be kept on file for a minimum of two years, or two years from the participants 18th birthday.
Should an incident occur during the event, be sure to report the incident to Security. Also, the following accident reporting form must be completed and distributed per the instructions on the form.
